Repeat Prescriptions

Using the Repeat Prescription Slip/ in writing

The PCT (health authority) requires all requests for repeat medication to be made in writing. Please use the right hand side prescription slip, ticking the boxes for the medication you require. Please post these in the white repeat prescription box in the surgery foyer. Some local chemists will collect repeat slips and deliver them to the surgery on your behalf.

By post:

Please send your request to the surgery address above. If you wish us to send your prescription back to you, please enclose an SAE.

Please

Please allow 2 working days for your repeat script to be processed once it arrives at the surgery. If the right hand side shows 'review overdue', the doctor will review your notes and the repeat prescription before reauthorising a repeat.
